Application, Registration, Emergency Information Card (including shot records and state-required lead screening tests) and YFC Food Program Enrollment Form must all be signed and returned prior to the first class for each student. These must also be updated for returning students. Permission slips for field trips will be issued as needed. No student may attend a field trip without a signed permission slip for that event.
